Individualised medicine, including biomarkers and gene expression profiles could help to decide whether radiation therapy is to be used and when. In fact, preoperative radiation therapy is becoming favoured in some clinical situations, especially since it allows a better understanding of its effects, such as exploiting possible immunological effects of radiation. Finally the role of combining systemic treatments and radiation therapy and its possible interactions will be addressed.
