Interestingly, a few cases (n=3) exhibited lower D max values in PBT_Com compared to PBT_Ave, primarily due to the relatively smaller contour in MBAS-based PBT compared to manual contour (Figure 2), highlighting the importance of fine-tuning the MBAS model.
Conclusion:
This study was the first comparison of PBT dose between contour on average intensity projection and combined multi-phase projection of free-breathing 4DCT using auto-segmentation in ultra-central lung tumours SBRT. Our results show that the PBT dose could be underestimated using the average intensity projection PBT contour. Future studies should focus on the fine-tuning of auto-segmentation models. Additionally, to further emphasize the significance of more accurate PBT dose estimation in SBRT planning, correlation of the increased dose in PBT_Com with clinical data, such as acute respiratory toxicity, is warranted.
