Patient plan was prepared for morning staff Treatment parameters transferred to both data bases (Varis and ExacTrac: labeled “ready for approval”) for QA purposes CTV was rejected and adjusted at morning staff This resulted in a change of isocentre co-ordinates Final plan was transferred to Varis R&V (labeled “approved by staff dd/mm/yy) … but not to ExacTrac

RTT discovered discrepancy in isocentre co-ordinates while dubble checking print out of treatment chart.
