Conclusions

• offline adaptive setup correction and margin adapting protocols with clinically proven benefits and high efficiency to reduce systematic errors

• online adaptive schemes can further reduce uncertainties for the price of more frequent imaging, but there is a lower limit for margin reduction

provocative thought: a single adaptation – if well implemented and its effects evaluated thoroughly – is better than some ‘latest’ complex IGRT/ART scheme with daily imaging and repeat adaptations – unless its benefits are clearly proven with treatment simulation approaches/patient data.
