Where does ‘patient specific QA’ fits into this scheme?

It is an additional QA element, if it is really patient specific. Hardware and software are just a part of the treatment chain. The bottom line is the dose in the patient .

It may be a useful redundant check, in particular in the early phases of clinical implementation.

It is a way to compensate for incomplete/unsatisfactory commissioning.
