2. Elective irradiation is becoming obsolete

• Head and neck cancer: dose de-escalation on the elective neck.

• Lymphoma: from mantle fields to IFRT to INRT.

• Breast cancer: accelerated partial breast irradiation (APBI).

• Esophageal cancer: no elective CTV in CROSS trial.

• Lung cancer: “incidental” nodal irradiation appears sufficient.

• Prostate cancer: 2 randomized trials on WPRT were negative.

Leukemia: from TBI to TMI.
