Recently… New identical Linac… Lessons: - Even in an institution with a lot of RTQA incidents can happen. - It is not sufficient to look at all steps separately, take an integral look at things. - Very detailed knowledge is required to implement the right RTQA procedures AND people should stricktly adhere to it.
