Radiotherapy or cystectomy in muscle-invasive bladder cancer: Outcomes with two clinical pathways

Purpose/Objective:

Bladder preservation (BP) and radical cystectomy (RC) with urinary diversion are both standard treatment pathways recommended for curative treatment of muscle invasive bladder cancer (MIBC). This study assessed the oncological outcomes of patients diagnosed with MIBC treated with curative intent with either of these modalities at a tertiary care centre.

Material/Methods:

Consecutive patients with histological diagnosis of urothelial MIBC, clinical stage cT2-T4N0M0, treated with BP or RC on the basis of patient preference, comorbidities, surgical fitness, and multidisciplinary tumour board discussion at our centre were included in this ethics-approved study. Patients treated with palliative intent, or those not completing the planned treatment were excluded from this study. The primary outcome was disease-free survival (DFS), and secondary outcomes were bladder cancer-specific survival (CSS), overall survival (OS), and bladder preservation rate in patients undergoing BP. Survival outcomes were estimated with the Kaplan-Meier method, and compared between BP and RC using the log rank test.
