For all patients, we gathered information on sex, age at diagnosis, cancer site based on ICD-10 codes, and the date of ACE-27 assessment. Residential locations at the time of diagnosis were used to infer patients' socioeconomic status, employing postcode mapping to the combined indices of multiple deprivation scores maintained by the UK Office for National Statistics. The prevalence of overall comorbidity scores was calculated using direct age-standardization of the cohort, stratified by deprivation index quintiles and age range (categorized as <40, 40-49, 50-59, 60-69, 70-79, and 80+ years old) to account for age differences in the population. Multinomial logistic regression was employed to investigate the association between cancer stage at diagnosis and various factors, including comorbidity levels, while controlling for disease site, age, sex, and socioeconomic status. Out of 80,212 patients, 46,530 (57%) received radiotherapy. Fron the full cohort, the median age was 68 years (range: 5-105), with 52.1% being female. Deprivation levels varied, with 29.4% in the most deprived areas and 17.9% in the least deprived. At least one ACE-27 comorbidity was present in 59.7% of the patients. The most common cancer types were breast (22.8%), digestive organs (18.6%), male genital organs (16.1%), and lung (14.9%). Skin, lung, and urinary tract patients tended to be diagnosed at older ages (median ages of 77, 72, and 72 years, respectively), while those with endocrine gland cancer were younger (median age of 51 years). Cancer groups exhibited significant ACE-27 score disparities (Figure 1). For instance, brain/CNS and endocrine cancers had the highest percentage of patients with no comorbidities (61.7% and 61.3%), while lung and urinary tract cancers had the lowest (21% and 30.9%). Lung and skin cancers showed the highest severe score percentages (19.7% and 12.4%), whereas endocrine and breast cancers had the lowest (1.8% and 2.6%). Results:
