Conclusion:

This novel IGRT driven OART procedure provides a robust means to account for daily anatomical changes, utilizing validated rigid registrations of both the prostate and bony anatomy. Our initial experience is that the complete workflow is feasible, and could be performed without the online supervision of a radiation oncologist. The algorithm can easily be generalized to other locations where differential movement of (semi) rigid structures play a role, e.g. lung, head and neck cancer or extremities. Going forward, we will focus on technical improvements to further reduce the fraction time and determine possible margin reductions.
