Conclusion:
The widely applied PTV margin of 5 mm proved sufficient to manage pelvic lymph node motion during CBCT-guided stereotactic radiotherapy, offering an opportunity to reduce this margin to 3 mm for most pelvic lymph node metastases. Importantly, the location of pelvic lymph node metastases was a significant factor influencing lesion motion and, in case of a pararectal localization, application of a larger margin or an online adaptive treatment approach should be considered.
