Two of the three functional parameters demonstrated a good separation between the RP and non-RP group with p values of 0.02 (significant) for Vent in the PTV and p=0.05 for Vent in the V20 without PTV region. The Vent in the whole tumor-bearing lung, the MLD and the V20 showed barely any trends between the two groups with p-values of 0.44, 0.84 and 0.78, respectively. The boxplots with the distributions of the best functional and the two clinical parameters are shown in Figure 1. These results coincide with the findings of the univariate ROC-AUC analysis (Table 1) where the Vent in PTV and in the V20 without PTV region performed best with median AUC-values of 0.83 and 0.79, whereas the MLD and the V20 performed worst with AUC=0.60 for both.

Conclusion:

Non-contrast enhanced ventilation imaging integrated in the clinical workflow at a 0.35 T MR-Linac requires no additional equipment and only short acquisitions, but revealed potential as biomarker for the early prediction of RP in lung cancer patients. Data from validation cohort is being collected.
