Conclusion:

RFs derived from baseline MRI imaging have the potential to aid prognostication in OPSCC. A model containing RFs demonstrated improved predictive capability compared with a similar model of standard staging parameters (T and N stage). Primary tumours which relapsed tended to be larger and display increased textural heterogeneity. Interestingly, two features IDMN and SDLGLE indicated increased areas of localised homogeneity in the relapse group, perhaps suggesting necrotic change or contrast pooling. The primary strength of this study is the well-described cohort. Limitations include the need for external validation and reduced model performance at higher specificities, likely related to the small number of relapse events. Radiomics remains a promising technique for early prognostication in RT treatment pathways.
