The first mitigation, ensuring bowel preparation instructions were provided in writing was introduced May 1, 2023. Printing on bright paper was introduced June 2023. A single combined audit of these interventions was undertaken between May and August 2023. The repeat audit included 21 patients. All had 5 # SABR treatments. Median treatment times over these patients was 57:58 min (range 31:98 min to 267:37 min). T# exceeded 60 minutes for 16 patients (76.2%) on 34 appointments and reasons are illustrated in Figure 2. In 27 of the 34 (79.4%) appointments either bladder or rectal inconsistencies were recorded as the reason for the prolonged treatment time, and frequency of needing to get off the couch during individual treatments is illustrated in Figure 2. Because this has not had an immediate positive result, from September 2023 a text message reminder service was implemented which informs patients 5 days prior to appointment to review the bowel preparation instructions. Additionally, a move to an adapt to shape workflow strategy has been proposed. This will allow a full plan adaptation based on the anatomy of the day and should mitigate the requirement for patients to get off the treatment couch to empty (or fill) their bladder or rectum with such frequency.
