Digital Poster DEVELOPMENT OF A QUALITY ASSURANCE PROGRAM IN A RADIOTHERAPY DEPARTMENT. María Jesús Macías Castillo, César Rodríguez Alarcón, Victoria Vera Barragán, Daniel Roberto Domínguez, Juan Bautista Quirós Rivero Radiotherapy, Hospital, Badajoz, Spain Purpose/Objective: The importance of Quality Assurance (QA) has been recommended by ESTRO due to the importance of a comprehensive program to detect and correct errors during the radiotherapy process. To improve the detection, classify, locate and solve errors in a radiation department an incident reporting system was established. Material/Methods: A QR code is provided in all units of the radiotherapy department are located. Any worker can report an incident that has taken place in the unit by a mobile phone device. They must anonymously answer 5 brief questions and suggest a proposal for improvement. The registered requests are sent to the e-mail address of the quality subcommittee team, which meets weekly to analyze all incidents and study possible corrective measures to prevent the problem from recurring. This subcommittee is made up of radiotherapy technicians, medical physicists, radiation therapists, supervisors nurses and administrative staff. Results: During September 2023 to September 2024, a total of 97 incidents have been collected out of a total of 1405 treatments realized. 54% of the incidents were in LA, and 35% during CT simulation process (Fig. 1).
Two of the errors were in relation to mistaken planification of treatments or with any error during the administration. Most of the incidents registered, 16 and 18, are in relation to administrative organization or misunderstandings between different workers (Fig 2.)
