The main measures carried out to improve have been: - Implementation of checklists of steps to be followed in the simulation CT. - Creation of “start-up RTT” whose major role is to review and prepare the initiation of patient treatments and to check that the treatment is in accordance with the established procedure. - Implementation of active patient identification methods. - Facilitating communication between workers and reducing the fear of expressing errors made. - Reorganization of the arrangement of the different immobilization elements and all the tools necessary for the treatments in the accelerator rooms. Conclusion: After the analysis of the incidences, it was found that most of them are of human origin (99.3%) and can be avoided. Errors in radiation therapy, with dose error up to 5% have also been reported in the literature. With the implementation of this system, we have acted on several of the weakest aspects of the radiotherapy process. In the same way that in other studies, in our experience, we can conclude that a quality assurance checking program facilitated the reduction of incidents with high dose errors. References: 1.
