Eligible: Low and Intermediate Risk Favorable High Risk (may have one of the following: Gleason 8-9, PSA <30, radiographic T3A) Ineligible: Very low risk and very high risk

DICOM treatment planning data will be collected for late effects modeling and correlation with subsequent clinical outcomes PROs, FFBP, OS, CTCAEv4 and ProCTCAE
