Left breast cancer radiotherapy:DIBH technique using BrainLAB Exactrac Dynamic versus free breathing

Purpose/Objective:

Adjuvant radiotherapy is a crucial component of breast conservation therapy that helps reduce the risk of local recurrence. In patients who receive radiotherapy for left breast cancer the long-term risk of developing heart disease is a major concern as the heart is in close proximity to the target area. Our research aim is to investigate the effectiveness of using the DIBH (Deep Inspiration Breath Hold) technique in left breast radiotherapy treatments with BrainLAB Exactrac Dynamic software. We intend to evaluate this technique in reducing the dose of radiation to the heart and LAD (Left Anterior Descending Artery). We will also measure the average treatment time and assess the patient's compliance with the deep inspiration treatment.

Material/Methods:

We examined 10 patients undergoing adjuvant radiotherapy for left breast cancer using Deep Inspiration Breath Hold (DIBH) technique. The radiotherapy was carried out using a TrueBeam STx and BrainLAB Exactrac Dynamic v.1.1.3. One CT scan was taken during free-breathing and another one during deep inspiration controlled using the Real-time Position Management system from Varian Medical Systems. Images were sent to Varian's ARIA oncology information system for contouring and radiation treatment planning was done using Varian's Eclipse treatment planning system, employing an IMRT technique and 6x energy. Contouring and planning were done on both free-breathing and deep inspiration images to compare the values of Mean Heart Dose (MHD), Heart V25Gy, LAD Mean Dose and LAD Maximum Dose.
