New developments in tumor imaging and radiotherapy delivery together with better understanding of tumor and normal tissue biology (sensitivity to high dose per fraction) allowed for safe hypofractionation in numerous malignancies. Level 1 evidence is available today for breast and prostate cancer and numerous studies are ongoing (early stage NSCLC etc.). The shift from conventional fractionation to moderate and extreme hypofractionation has been introduced in daily practice. How far can we go with hypofractionation? The biological mechanisms must be investigated in depth in order to understand if, when and how extreme hypofractionation or even single-fraction radiotherapy might be applied.
