PCNSL: A unique treatment challenge • Rapidly lethal if not treated or responsive • RT alone is effective , but CRs are brief (median survival– 1 yr) • Adding CHOP or CHOD does not improve RT alone results • High-Dose Methotrexate is the mainstay of effective treatment best for inducing a complete response • There may be a small benefit from adding rituximab • High-dose ARA-C has also been used for consolidation and salvage • High-dose chemotherapy with autologous stem cell rescue has been suggested as an alternative to RT consolidation for fit patients
