ICHNO-ECHNO 2022

nasopharynx in a region known to be endemic for this type of tumor.

Materials and Methods Descriptive, cross-sectional study in 214 patients with nasopharyngeal cancer treated at the level of the Radiotherapy department of the regional center for the fight against cancer of Batna, Algeria over a 24-month period from January 2015 to December 2016 for a general population of 5.9 million inhabitants

Results First ENT cancer with 214 cases - The fifth of all cancers. - an estimated gross incidence at 1.81 cases / 100,000 habit / year

- The sex ratio = 2.1 (145 men / 69 women).

- The average age is 46 (6 to 88), the most affected age group is 40-50 years.

- Classic risk factors, diet rich in nitrosamine, socio-economic level modest are almost constant. The notion of family cases is mentioned in only 3 cases. Only 5 patients were able to have EBV viral serology all positive. - Undifferentiated squamous cell carcinoma type 3 (UCNT) represents alone 91, 3%.

Tumors of the side walls are the majority (68.6%) often associated with cervical lymphadenopathy; against 29.8% of the posterior superior wall with neurological impairment.

- Nasal obstruction and cervical lymphadenopathy are the 2 main reasons for consultation in 33% and 30% of cases, respectively, followed by headaches (23%) and hearing problems (12%) - A systematic assessment for all patients: complete clinical examination, a CT and / or cervico-facial MRI, a TAP and a bone scan in addition to a general assessment.

- Stage III is the most common (33.7%), followed by STAGE VI A (29.6%); Barely 0.6% of stage I; M STAGE + 13% (89.4% bone); A diagnostic and therapeutic delay estimated at 8.7 months

- The therapeutic modality adopted given the importance of locally advanced cases is often neo-adjuvant chemotherapy (TPF or CDDP / DOXO) followed by RCC (CDDP 100 D1 / D22) according to 3D techniques (50gy prophylactic / 70gy GTV T / N) with weekly monitoring. 92% complete remission at 6 and 12 months on imaging and caval biopsy; only 17 failed cases with 9 lymph node remnants recovered by dissection. However, 29% of patients developed metachronous metastases, especially bone (74%); initial lymph node involvement and T4 tumors as well as young age are the 3 predictorsof poor prognosis.

Conclusion Nasopharyngeal cancer is endemic in our region, affecting young working adults; It is a curable cancer thanks to modern radio-chemotherapy combination with tolerated toxicities. It remains to improve diagnostic and therapeutic times.
