Advanced-fractured osteoradionecrosis in long-survivor ENT cancer patients: a medical challenge?

Purpose/Objective:

Mandibular osteoradionecrosis (ORN), limited in frequency today, is described in the literature on series of 22 to 87 patients, of which 60% of advanced cases including mucosal defect > 2.5 cm (or length > 2 teeth) and radiological depth of bone lysis beyond the inferior alveolar canal, with severe complications such as fracture, osteomyelitis, fistula, lower dental neuropathy. The treatment of these patients suffering with multiple co-morbidities (diabetes, neutropenia, HIV, ..) is always complex. Radical surgery as hemi-mandibulectomy - osteocutaneous free flap reconstruction is an option when the risk of serial postoperative complications is not too high.
